Welcome!  Please complete this form to reserve a swab slot for RT-PCR or Rapid Antigen. Details of your reserved slot will be emailed to you within the day if your request was received from 8 a.m. to 4 p.m. All requests received after cut-off will be attended to the next day, during business hours.  

Swab services are available Monday to Saturday, from 7:00 a.m. to 11:00 a.m.; 12:00 p.m. to 3:00 p.m.
