為個人資料之保護，依照個人資料保護法第8條規定進行蒐集前之告知:
To protect the personal data of the participant, we hereby inform the participant the following terms in accordance to Article 8 of the Personal Data Protection Act:

一、個人資料蒐集之目的及用途 Purpose and method of collection:
目的在於進行本次論壇活動辦理之相關作業，主辦單位並將利用您所提供之Email及聯絡電話通知您活動之相關訊息。
Your personal data is required in order to conduct event organization processes, the participant's email address and phone number will be used by the event organizer for event-relevant announcements.

二、蒐集之個人資料類別 Classification of personal data to collect:
1. 中文姓名 Participant's name
2. 聯絡電話 Participant's phone number
3. 電子郵件信箱 Participant's email address
4. 服務單位 Participant's organization
5. 主辦單位因活動需求所需收集之資料，由主辦單位另訂之。Any requirement of personal data not stated above will be
    announced by the event organizer accordingly.

三、個人資料利用之期間、地區、對象 Duration, region, and target of personal data collection/utilization:
1. 期間: 您同意參加本次論壇之日起，至活動結束後一年為止。期間由主辦單位保存您的個人資料，以作為本人、主辦單位查詢、確認證明之用。
     Duration: The participant agrees that his/her personal data be stored by the event organizer, for the participant and
     organizer's referencing and confirmation usage, within a duration starting from the participant's successful registration until
     one year after the event has ended.
2. 地區: 您的個人資料將用於活動主辦單位提供服務之地區
    Region: Your personal data will be used in areas where the event organizer provides its services.
3. 對象: 透過網路報名系統之使用者
    Target: Participants registered through this online registration form.

四、依據個資法第3條規定，報名者對個人資料於保存期限內得行使以下權利 According to Article 3 of the Personal Data Protection Act, the participant may exercise the following rights with regards to his/her  personal data:
1. 查詢或請求閱覽 the right to make an inquiry of and to review his/her personal data;
2. 請求製給複製本 the right to request a copy of his/her personal data;
3. 請求補充或更正 the right to supplement or correct his/her personal data;
4. 請求停止蒐集、處理或利用 the right to demand the cessation of the collection, processing or use of his/her personal data; and
5. 請求刪除 the right to erase his/her personal data.

五、提醒 Attention:
您可自由選擇提供個人資料，若其提供之資料不足或有誤時，將可能導致無法成功報名此次活動。Participants may decide which personal data to provide, however, registration may be affected should the said personal data be incorrect or insufficient.

(Please be aware that the agreement to this "personal data collection consent" is equivalent to that of a written consent of the Personal Data Protection Act.)

同意，代表您已閱畢本次活動報名之個資告知事項，並願以電子文件方式同意主辦單位蒐集、處理及利用您的個人資料。
By agreeing, you have read the above terms in detail, and agree to the collection, processing, and utilization of your personal data by the event organizer through electronic documents.
不同意，代表您不同意主辦單位於本次活動蒐集、處理及利用您的個人資料，請關閉此頁面，此次報名將不會成功。
By disagreeing, you do not agree to the collection, processing, and utilization of your personal data by the event organizer.  Please exit this page and your registration will not be counted.
