If your visitors have not made a decision to receive Christ during this first-time visit, please provide only the number of visitors.

If your visitors prayed to receive or re-dedicate their lives to Christ on a separate occasion, please resubmit this form to indicate the number of decisions, and in the field  "Number of First-time Visitors", indicate “0”.
