This form must be completed by the Contact Presenter, who is responsible for keeping co-presenters informed about the status of the proposal. The Contact Presenter must supply institutional addresses and emails. This person also must inform the Program Chair of any address changes or other variances related to the proposal. An acceptance confirmation is emailed only to the Contact Presenter listed on the Call for Proposal Submission Form.

Abstract - In 250 or fewer words, describe the objectives and content of the presentation. Include   the intended audience level (new, intermediate, advanced), the interest strand(s) targeted, and what learning is intended for participants.  Describe the type of presentation (demonstration, paper, workshop, SIG) and any research relevant to the presentation – research may be in the form of a bibliography. 

Summary - Type a 50-word (maximum) summary.  This is a blind page and is NOT to include the name(s) of the presenter(s).  If the proposal is accepted, this summary will appear in the symposium program. *
