When fostering pets from a high-intake facility, it is very important that all of your pets at home be up-to-date on all vaccinations and boosters, for their protection. This includes Rabies, DA2PPV / FVRCP (including boosters) and Bordatella (Kennel Cough)

I understand that all animals fostered remain the property of the LOGAN COUNTY HUMANE SOCIETY (LCHS) and must be adopted through the LCHS application process. I understand and agree that I may not transfer or adopt out my foster animal to any other person, family, rescue, shelter or organization without the prior written permission of the Logan County Humane Society. Any friend or relative of a foster parent who wishes to take over my foster or to adopt my foster must apply and meet the same criteria as any other adopter.

By checking "I Agree" below, I agree to take proper care of my foster animal, providing appropriate food, shelter, bedding, exercise, medical care, training, love and affection, to keep in regular contact with LCHS, report any problems, medical issues or symptoms immediately, and return the animal to the LCHS for any medical appointments or treatments required. 

By signing this I understand that failure to comply with any of these conditions will hereby terminate my status as a foster parent. 

In agreeing to this Foster Parent Agreement I  hereby acknowledge that by bringing into my household animals that are or may be sick or injured, there is a risk to my own animals, even though they have been properly vaccinated. By signing this, I agree not to hold LCHS liable for any illness or injury caused as a result of any animal I am fostering.
