1. Sex. Sex refers to the biological and physiological characteristics you are assigned with at birth. These biological characteristics include genitalia, hormones and chromosomes. Please let us know what sex you are.
2. Gender. Gender is a social construct and refers to the attitudes, feelings, and behaviors that a given culture associates with a person’s biological sex. This can vary from culture to culture, therefore gender is a fluid concept. Behavior that is compatible with cultural expectations is referred to as gender-normative. Male and female genders sit within what is called a binary concept of gender. There are many non-binary concepts of gender too- see below. Which gender do you self-identify as?
3. Age. What is your legal age as stated on your passport or birth certificate?

4. Nationality or Citizenship. Your nationality/ citizenship is the country in which you are born and/or the country where you have legal citizenship status. It is a legal concept, where you are bound by the laws of that country and have a passport for that country. You can be a national of more than one country. Which country or countries are you a national of?
5. Ethnic Origin. Your ethnic origin refers to your racial ancestry. Ethnic groups are united by common traditional, cultural, linguistic, ritualistic, behavioral and religious traits. Please tell us your ethnic origin.
6. Religion, beliefs and spirituality. Religion is a set of beliefs and practices that help people connect with God or a transcendent entity. Spirituality is a broad concept that can mean the search for transcendent meaning, an expression of the sacred, the belief in the relationship between ourselves and something larger and interconnected. Please tell us your religion, belief system or expression of spirituality if you have one.
7. Sexual Orientation/Expression. Sexual orientation is used to describe the gender(s) someone is usually attracted to. A person’s sexual expression can be fluid and change over time. What is your sexual orientation?
8. Disability. Under the Equality act 2010, disability in the UK is defined as having a physical or mental impairment that has a ‘substantial’ and ‘long-term’ negative effect on your ability to do normal daily activities. Do you consider yourself to be disabled, to have an impaired health condition or learning difference?
9. Have you attended a Higher Education institution? (even if you didn’t or haven’t fully completed)
10. Do you identify strongly with a political party? If yes which one?
