5/10/25 Healthy Mind Healthy Ocean, AquaMeridian Beach Cleanup Volunteer Registration and Liability Waiver

Join us on May 10, 2025, for a beach cleanup in honor of Mental Health Awareness Month. Connect with nature, give back, and uplift your well-being while protecting our coastal environment. 

The garbage piling up along the Seaplane Lagoon shoreline threatens wildlife, water quality, and overall ecological health. With the support of our dedicated volunteer community, we can make a meaningful difference by removing debris and restoring this vital habitat.

We offer volunteer hours to high school and college students and warmly embrace corporate volunteers to join us in strengthening our student-run foundation and the broader community. Families are also encouraged to participate in our efforts. Protecting the environment is a goal we all share, and by engaging in these activities, we set a positive example for the next generation.

BEACH CLEANUP 2025 WAIVER OF LIABILITY AND EXPRESS ASSUMPTION OF RISK (PLEASE READ CAREFULLY)

I agree to the following terms for my participation in the Waterfront Park beach cleanup on March 8th, 2025:

  1. I will perform tasks within my physical capabilities and avoid those beyond my ability.
  2. I will not participate while impaired by alcohol or drugs.
  3. I will only use machinery, equipment, and tools I am familiar with and can operate safely.
  4. I will follow provided instructions, safety procedures, and use supplies from the event organizers.
  5. I will adhere to assigned tasks, safety rules, and perform my duties with care.
  6. I acknowledge my volunteer status and waive any claims for compensation or insurance from government agencies or event organizers.

I am aware that beach cleanup involves inherent risks, such as injury from hazardous materials, wildlife encounters, environmental conditions, or potential watercraft use risks. I willingly choose to participate, understanding these risks and having no known physical limitations. In case of injury, I authorize any licensed California physician to provide necessary emergency treatment.

I confirm my legal competence to sign this binding document, having reached the age of eighteen or obtained consent from my parent or guardian. I understand that this agreement is a legally binding contract, and I sign it willingly and of my own accord.

I agree to allow my image to be used in published materials and websites that promote the programs of AquaMeridian US and/or any programs conducted by the Released Parties. By including my email address below, I understand that AquaMeridian US may contact me about future beach cleanup events and other programs.

I HEREBY RELEASE AND HOLD HARMLESS THE RELEASED PARTIES FROM ANY CLAIM OR LAWSUIT FOR PERSONAL INJURY,PROPERTY DAMAGE, OR WRONGFUL DEATH, BY ME, MY FAMILY, ESTATE, HEIRS, OR ASSIGNS, ARISING OUT OF PARTICIPATION INTHE EVENT, INCLUDING BOTH CLAIMS ARISING DURING THE ACTIVITY AND AFTER I COMPLETE THE ACTIVITY, AND INCLUDINGCLAIMS BASED ON NEGLIGENCE OF OTHER PARTICIPANTS OR THE RELEASED PARTIES, WHETHER PASSIVE OR ACTIVE.

I HAVE FULLY INFORMED MYSELF OF THE CONTENTS OF THIS LIABILITY RELEASE AND ASSUMPTION OF RISK.
