Renal Care Questionnaire
Are you a nurse working with patients with kidney disease, kidney transplant, or other renal care needs? If so, we would value your opinions to help us identify aspects of renal care for which we may be able to develop technology solutions that will help improve patient care, experiences and outcomes.

About us:
The National Institute for Health Research (NIHR) Devices for Dignity MedTech Co-operative (D4D) is based within the NHS and supports the development of health technologies to support patient need. You can learn more about D4D and how we work with healthcare professionals to develop medical technologies on our website, www.devicesfordignity.org.uk.

This survey has been developed by Nurse Consultant Dr Helen Hurst, expert patient David Coyle, innovation expert Dr Katherine Jeays-Ward, and Consultant Nephrologist Professor Sandip Mitra. Any information you provide will be treated anonymously.

The survey will take around 5 minutes to complete, and is best completed in Chrome or Safari.
