Hello Gilmore families. My name is Joy Nocerino and I am the school counselor. You will find more information at the purpose of this assessment below.

PURPOSE OF THIS SURVEY:
The purpose of this survey is to help me learn about the challenges families are facing during this time.

HOW WILL THIS SURVEY BE USED?
I will use the results of this survey to determine which types of services I will need to provide to families. In addition, I may use the results of this school-wide survey to report out various whole-school statistics to raise awareness on major issues families are facing.

IS THIS SURVEY ANONYMOUS?
Yes! Your responses on this survey will NOT be used in any way to identify you, unless you choose to put your personal information in, which is optional. They will not be tracked, nor reported. Please be honest, since the results of this survey will affect the counseling program at our school.
