Would you like assistance in booking your COVID-19 vaccine appointment?

If you are currently living in Ontario and 12 years of age or older, you are now eligible to receive the COVID-19 Vaccine. You do not need an OHIP card and there is no charge, as the government is covering the cost of the vaccines.

If you would like assistance booking a vaccine appointment, please submit this form with your information and one of our staff at UMC High School will contact you to arrange an appointment close to your home.

The available vaccines are Pfizer (for 12 yrs+) or Moderna (for 18 yrs +) and require 2 doses. All students in Ontario will receive both doses by the end of August in order to be fully vaccinated by the start of the new school year.
