CONSENT & ACKNOWLEDGEMENT FORM

As a student and parent in membership with the 2024-2025 Chevaliers, we have read the Drill Team Constitution and understand that our student will be governed by the above rules and requirements. Failure to do so may mean dismissal from the drill team. We are fully aware of the responsibilities of a Chevalier. We realize that there are financial and time commitments that must be made in order for a team and program to be successful. We understand that the drill team is a UHSAA varsity sport and therefore there will be rehearsals held during summer months, during school, and outside of the school day.

We agree that we will not in any way hold Alpine School District or its institutions/employees responsible for any harm or injury resulting from our student’s participation in this activity. We also understand that our insurance will be the prime carrier in case of injury or accident directly involving our student.

Membership as a Chevalier requires total commitment and our signatures indicate that we are committed to support the Drill Team and Lone Peak High School.
