SickleCellMN COVID-19 Financial Assistance Form
Sickle Cell Foundation of MN (SickleCellMN) recognizes the enormous burden of COVID-19, especially for those living with a chronic illness such as Sickle Cell Disease (SCD). We also recognize that many either CANNOT work or SHOULD NOT work because of compromised immune systems and that this creates added financial worries for many.
Sickle cell patients and caregivers are encouraged to FIRST utilize/reach out/exhaust all federal and state resources. SickleCellMN continues to identify resource gaps and will fill community needs as we are able. Thank you for you on-going patience and support.
Individuals and caregivers are eligible to receive a one-time grant up to $250 per household.
PLEASE NOTE: As a non-profit organization, we rely on the generosity of our sponsors. Program continuation is contingent on the availability of funds. The program could be modified or discontinued at any time if funding is limited or no longer available.
-- Applicant /Patient must be a FULL TIME RESIDENT in the state of Minnesota (Proof may be required)
-- Patient(s) must have a confirmed sickle cell disease diagnosis, be in active treatment, scheduled to begin
treatment or in follow up care, all attested by the patient or care team member.
-- Patients 18 and over must self-apply as an adult living with SCD (even if still being seen in pediatrics)
-- There is no income criteria.
-- Patients may be insured or uninsured.
-- Patients do not need to have a COVID-19 diagnosis.
