This form should be used by Minerva faculty and staff to share situations that may need a coordinated effort from the
Community Care Initiative (CCI) team. After we review the submission (generally within 24hr, but often much faster), it is standard practice for us to reach out to you, while we also consider other things the team can do to help.
This form is NOT appropriate for urgent or emergency level situations. In those cases, please explore other options such contacting staff members directly or calling someone for more rapid assistance. You can find a listing of some of those
resources here.
