I give permission for the staff and volunteers of First Presbyterian Church of Whippany to photograph or videotape me, my family and my child/children for purposes of in-house church use and/or for public information for promotion of the church (i.e. brochures, websites, newspapers, radio, television) with the promise of no child(ren)s names will ever be released!

I agree to indemnify, hold harmless and promise not to sue the First Presbyterian Church of Whippany and its leaders, employees, volunteers, or agents from any and all liability, actions, causes of actions, claims, expenses, obligations and damages of any nature whatsoever arising from my family's participation in its activities and programs, or as a result of injury or illness of me, my family or my child(ren) during any activities. It is understood and agreed that this is a full and complete release and waiver of all claims and damages which the undersigned may claim as a result of participation in said activities by reason of injury, negligence, or other cause,and all losses as a result thereof. It is further understood and agreed that in the event that any claim is asserted against Releasee, I will hold Releasee harmless from such claim.
