Directions: Please answer each question by circling the number that corresponds to your level of disagreement/agreement with each statement. The rating scale ranges from “Strong Disagree (1) to Strongly Agree (4), place a check in the circle if you never received the information or circle N/A if you have no opinion on the statement. We appreciate your feedback!

Section 1 – To date, have you received information from your child’s school about awareness of standards and testing?
