Mammoths Scholarship Application
Complete this form to apply for the Kenosha Mammoths Rugby Scholarship.
Requirements are as follows:

1.     Attend Practices: Attend 14 practices over seven weeks on Tuesday and Thursday evenings from 7:00 to 8:30 pm.  Starting February 20th.

2.     Attend Matches: Be present at 5 matches on Saturday afternoons.

3.     Campus Promotion: Actively promote rugby on the UW Parkside campus.

4.     Good Citizen and Teammate: Be a good citizen and teammate both on and off the field.
