By registering my child/children and signing this form, I, consent to our child/children participating in the activities connected with trips to and from and the time during Family Conference Babysitting at Logos Baptist Ministries. I certify that my child is able to participate in all activities scheduled for Family Conference Babysitting (unless otherwise indicated). If my child has medical conditions which may be relevant to a physician in the event of an emergency, I have listed them above. In the event an emergency occurs, I may be reached at the telephone number(s) listed above. If I cannot be reached within a reasonable period of time, I hereby authorize the emergency contact listed above to make emergency medical decisions for my child. If there are any activities I do not want my child to be involved in, I have listed them above.

I hereby authorize LOGOS BAPTIST MINISTRIES to take or permit pictures to be taken of my minor child/children for use by LOGOS BAPTIST MINISTRIES in publications, church bulletins, newspapers, newsletters, its website, and/or on social media platforms for purposes of public relations for LOGOS BAPTIST MINISTRIES. LOGOS BAPTIST MINISTRIES shall also have the right to use any portion of any statement made by my child in any publication, newspaper, newsletter, or website. This agreement contains all the understandings, oral and written, of the parties and supersedes all previous agreements. If any portion of this Agreement is found to be invalid or unenforceable, it shall not affect the balance of this Agreement. This Agreement will be governed by the law of the State of ILLINOIS.

I understand and hereby agree to assume all of the risks which may be encountered during said activities occurring during Family Conference Babysitting, including activities preliminary and subsequent thereto.

I do hereby agree to hold LOGOS BAPTIST MINISTRIES and its agents and employees, harmless from any and all liability, actions, causes of actions, claims, expenses, and damages on account of injury to my child or property, even injury resulting in death, which my child now has or which may arise in the future in connection with the activity or participation in any other associated activities. I also specifically agree that LOGOS BAPTIST MINISTRIES shall not be responsible for any such injuries, loss or damage even in the event of negligence or fault by LOGOS BAPTIST MINISTRIES.

I understand that LOGOS BAPTIST MINISTRIES does not provide medical coverage for my child. I verify that I will be responsible for any medical costs that my child may incur as a result of their voluntary participation, under my supervision.

I expressly agree that this release, waiver and indemnity agreement is intended to be broad and inclusive as permitted by the law of the State of ILLINOIS and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. This release contains the entire agreement between the parties hereto, and the terms of this release are contractual and not a mere recital.
