Hello, My name is Madison Milbrath and I am a graduate student at North Dakota State University. I am interested in studying how various regions across the state of ND interact with informal STEM education opportunities, such as STEM kids. The survey should take approximately 5 minutes.

I request your participation in a study about STEM kids participation by region across the state of ND. By completing this survey you are giving your permission to use related answers as data in this study.

Participation in this study is completely voluntary. If you decide not to participate there will not be any negative consequences. Please be aware that if you decide to participate, you may stop participating at any time and you may decide not to answer any specific question.

The researcher will maintain the confidentiality of the research records or data.

If you have any questions, please contact me at madison.milbrath@ndsu.edu Additionally, you should know that your child(ren) is/are also being asked to participate in a survey. The survey for children will be contributing to the same research study as yours. The children survey will ask a child how they feel about science and math in a variety of questions. Their information will remain entirely confidential. If you do not feel comfortable having your child participate know that they do not have to, and will not be penalized in any way for not participating in it.
