TD4L Class Registration Form
Please complete the form and you will be sent an email an hour before class start from your instructor with the Zoom log in information and password in order to join class. We mandate registration for our participant's safety to minimize any instance of online threats that would disrupt our classes and compromise security overall. You will receive a confirmation of your registration via email after you submit the form.
Registration WILL close an hour before the class start. So the recommendation is that you register even if you are unsure if you will make it to class or not 🙂 There is no penalty if you need to skip out. PM Triangle Dance 4 Life Facebook page or email us @

OPTIONAL FIELD - We recognize that these are tough times for all, and our group's intention is to continue to help you reach your fitness goals, and hopefully bring a bit of joy and stress relief. For this reason, we are not asking for payment for classes, but we are providing the option to donate (your choice of amount, max of $5), if you feel inclined to do so.
