Purpose: Parent Permission Form assuming responsibility to Transport their Student, and ONLY their student, to/from an TH Bell Event on their own.

Consent and Acknowledgement: I, hereby acknowledge that I have carefully read and acknowledge my understanding of this Parent/Guardian Release Form and the Hold Harmless Agreement found below.  As the parent/legal guardian of the student named below, I hereby grant permission for my student to be transported TO/FROM the TH Bell field trip/event by myself or another adult appointed by myself.  I understand that TH Bell offers transportation to students by bus and that my student may travel to this event on a school bus.  The vehicle in which I travel/drive has insurance and I understand that this insurance policy is the prime insurance carrier. I further acknowledge that this waiver relieves Weber School District of any responsibility and/or liability to verify my private insurance. 

I assume all liability for my student during the transportation to and from the event.  I understand that by traveling to/from the above named event by private vehicle, Weber School District, its institutions or employees are in no way responsible for any harm or injury resulting in travel to and from the event. 

I understand my student will still need to check-in with their teacher/coach immediately when they arrive at the event, and check-out with their teacher when leaving the event with their appointed transportation

RELEASE

I hereby release the high school and Weber School District (and its officers, directors, shareholders, members, managers, agents and representatives) from any liability associated with any harm or problem which may occur in the transportation to and from the event.

Hold Harmless Agreement 

I hereby waive, release, discharge and hold harmless TH Bell Jr High School and Weber School District, along with their staff, from and against, without limitation, any losses, damages, claims, demands, actions, rights, costs and expenses, including reasonable attorney’s fees, that may arise from the personal transportation, including but not limited to personal injuries, death and disability.
