I, the undersigned, am the parent/legal guardian of the below named child(ren) who is (are) a participant in the Community String Project. I understand that my child(ren)has been granted permission to use a CSP instrument, including the bow and the case, for use in the program. I also understand that this is a delicate instrument and that special care will need to be taken to protect the instrument. In consideration of my child(ren) being permitted to use the instrument, I agree to accept full responsibility for the care of the instrument. Further, I agree to accept full financial responsibility for its replacement value (up to $400 for violin, $465 for viola, $850 for cello, $1700 for bass) should the instrument be broken, lost, stolen or, in CSP’s estimation, irreparably damaged during the period that it is loaned to my child. I also understand and agree that if the instrument is not returned by the agreed upon return date, the end of the current CSP season, or upon leaving the program, it will be considered lost and I will have full financial responsibility for the replacement value. Additionally, I agree not to permit anyone else to use the instrument during the time period that it is signed out in my child’s name and to take necessary precautions to ensure that my child does not allow anyone else to use the instrument. I acknowledge that I have read and understand the terms of this agreement and that I am of legal age to bind myself to this agreement. This agreement has been executed on behalf of myself, my heirs and assigns. This agreement has been executed in and shall be interpreted according to the laws of the state of Rhode Island.

COZ Program For Rockwell School only
This year CSP will offer complimentary child supervision through the COZ program at Rockwell School during the gap time between the end of school and the beginning of CSP Classes. Supervision will not be offered after CSP instruction at all schools unless you are registered with the COZ program at that school.

Please read and go over the below family contract with your child. Checking the below box indicates that you and your child understand and will adhere to all terms of this family contract. This contract is also available on our website in the "Forms" section under "Program Info".

The Community String Project is here to enrich your child’s and your family’s life through a
program of events, including lessons, events, and performances. To ensure that the program
will do the most for your family, please review and remember the following list of CSP
expectations. Your child will be expected to adhere to the following terms of CSP membership, which will insure their success as a developing musician and member of their school and community.

Respect:
• My child will be respectful towards all those involved in the program: fellow students, family
 members, teachers and other participants.
• My child will be respectful of the spaces we use.

Instrument care:
• My child will take care of their instrument and bow by handling it the way my teacher has taught me.
• My child will not let others handle or play their instrument.
• My child will keep their instrument safe by never leaving it in the car, keeping it away from extreme hot
  and cold areas, and away from pets.

Practice and Preparation:
• My child will come prepared with their instrument and any materials needed for each lesson.
• My child will commit to practicing their instrument at home at least five days a week.
• My child will arrive on time with clean hands and short nails.
• My child will not use electronics, such as cell phones or video games, during lessons or CSP events.

Attendance:
• We agree to attend all lessons and performances on time.
• More than three absences in a semester will result in a meeting with my child’s teacher.
• We agree to call the child’s teacher if we will not be able to attend a lesson.

Support
As a parent I agree to support my child in his/her involvement with CommunityString Project. This includes facilitating daily practice sessions, and providing an area suitable for quiet practice. I also agree to take responsibility for the accompaniment and supervision of my child at CSP events including performances and before and after lessons.

These are the policies and expectations of our program. If recurring problems arise, your
teacher may request a meeting to discuss them with you. If problems continue to persist after parent/student/teacher conference, CSP retains the right to suspend the student from CPS participation until all issues are resolved.
