This form is reserved for use by MWC student-athletes who will be or have been compensated for the use of their names, image and likeness for promotion purposes and wish to use the MWC logo, mark and/or related branding as part of their NIL activities. All other inquiries about the MWC logo, mark and/or related branding should be submitted via email to the MWC Executive Director.

The MWC logo, mark and/or branding may NOT be used by student-athletes or prospective student-athletes as any part of any NIL agreement without prior written permission from the MWC Executive Director.
