Luna Paws Adoption Application
Thank you for your interest in adopting one of our cats! Please take a moment to read our adoption agreement and disclaimer.
Please fill out the following cat adoption application carefully and completely. This information will help us make the best placement possible. Our goal is to find a loving, responsible, and permanent home for the cats/kittens in our care. It may take 24–72 hours to verify the information you have provided at which time we will reach out to you. Failure to fully and accurately answer questions may cause a delay or denial in your application process. If your application is approved, a “meet and greet” will be scheduled so that you/your family can meet and interact with the cat.
The prospective adopter agrees:
1. Cats are to live in a private residence as companion animals.
2. To provide the cat with sufficient quantities of nutritious food and fresh water each day.
3. Never to strike or otherwise harm the cat.
4. Never to have the cat declawed, unless medically necessary.
5. Spaying or neutering is required by five months of age (if not already fixed). Proof of surgery must be mailed/emailed to Luna Paws within 30 days of the procedure.
6. To ensure that the cat’s vaccinations for rabies and distemper are current and to provide veterinary care
upon sickness, disease or injury.
7. If the cat must be relinquished for any reason by the adopter, s/he MUST NOT turn the cat
over to a humane society, shelter, person or let loose outdoors, but must return the cat to Luna Paws. The adopter understands Luna Paws will not refund the adoption fee if the cat is returned.
8. Luna Paws may examine and make inquiry about adopted animals at any time, including home visits if necessary (ex. if abuse or neglect is reported). Upon finding that the conditions set forth in this contract have been violated, or if there has been a failure to follow relevant animal ordinances or cruelty laws, the animal may be returned to Luna Paws, its officers or agents.
9: The adopter understands that animals are unpredictable and that Luna Paws cannot anticipate or ensure against unexpected conduct of cats adopted. The adopter acknowledges that Luna Paws has not made through its agents, volunteers or employees, any warranties regarding the future condition, temperaments, or conduct of the animal. The adopter hereby accepts the cat as/is, assuming all risks and responsibilities associated with ownership of the animal, including zoonotic diseases, scratches, and bites. The adoptee hereby fully and completely releases, and indemnified, and hold harmless Luna Paws directors, officers, volunteers, agents and employees from any claim, cause of action or liability of any sort of nature arising out of the adoption, care ownership, maintenance, or condition of the animal.
10: The adopter understands that failure to perform the foregoing agreement will constitute a breach of contract.
Disclaimer:
We recommend you consult your veterinarian on appropriate precautions to protect your current pet(s) during their introduction to your new pet. Luna Paws is unable to treat your existing cat if they are exposed to any illness by your newly adopted pet(s). Luna Paws strongly encourages you to have your new cat examined by a veterinarian within 14 days of cat adoption. Luna Paws cannot reimburse you or your veterinarian for any medical expenses that you may incur. As the adopter, you agree to take full responsibility for the future care and well-being of your pet, including all medical bills and decisions.
