If you have recently moved into our district, or had an addition to the family (such as birth or adoption), please complete the following form.  This will ensure you receive information on Preschool Health Screening, Early Childhood Family Education programs, Kindergarten Roundup/Registration, etc.

Please only include children ages infants through 6 who are not yet enrolled in elementary school.
