Please give NAME, EMAIL, and PHONE for a music teacher who can speak to the student's basic pitch matching with their singing voice. (This is may not be strictly required if you have selected Drama only, or if you are signing up as an exclusively instrumental performer by advance agreement with the director.) *
If signing up by prior agreement with the director as an INSTRUMENTALIST or as a VOCAL SOLOIST, what availability does the student have to learn some music in advance in July-August? (For example, if the student needs to look at music in advance but will be in a full-time music camp for two weeks immediately prior, we need to know that we'd have to send them material earlier in the summer.)
