Optometrist Survey
Dear Colleague

You are invited to participate in a study titled: “The environmental pathway and impact of plastic lenses used in Optometry.” This research is being conducted by Rayishnee Pillay, a student at the Department of Optometry, University of KwaZulu-Natal (UKZN). The purpose of this survey is to determine the views and recommendations of optometrists regarding the disposal of spectacle lenses and contact lenses. The study would like to establish the effect of lens disposal on the environment and ultimately make recommendations on best disposal practices for the environment.

This study has been ethically approved by the UKZN Humanities and Social Sciences Research Ethics Committee, Approval Number: HSS/1649/018D. It should take approximately 6 minutes to complete this questionnaire. Participation in this research is voluntary and you may withdraw at any stage. There are no costs that will be incurred by you nor any incentives for participation in the study. All information will be completely confidential, only used for research purposes and accessible to the primary researcher. All electronic information will be stored in a password-protected computer and will be deleted five years after completion of the study.
