As a part of the collective efforts by the TAC to better serve the Colorado Tibetan community we are requesting that you fill out this Colorado Tibetan community informational survey below. Without knowing who, where and how many we are serving it becomes an increasingly difficult task to contact, organize or share important information with the community. With this informational survey, the data obtained will enable the TAC to effectively implement and manage general tasks as well as provide insight into where or how the TAC can better serve the Tibetan community in Colorado.

Notes:

1. Please use the exact name as it appears on your government issued ID.
2. Please fill this out as a family unit if possible. If you enter your spouses information in section 2 they do not need to complete a separate survey.
3. A child is a minor who is younger than 18. If you are an adult 18 or older please fill out an separate survey.
4. Your personal data is completely confidential and will only be used within the premises of better serving our community members by TAC.
