71st Annual Crime Prevention Awards Nomination Form
LISC Milwaukee and Safe & Sound are proud to host the 71st Annual Crime Prevention Awards Wednesday, February 5, 2020 from 11:30 PM - 1:30 p.m. at the Wisconsin Club.

If you know members of the community, law enforcement or organizations whose crime prevention efforts have benefited the Greater Milwaukee community, please submit their names for a nomination to the appropriate award category.

Please note, you may only select one award category per nominee. Multiple nomination forms are accepted.

Nominations are due Tuesday, November 26, 2019.

Community Awards- Given each year to a community member or group who meets the qualifications below.
AAA Wisconsin Traffic Safety Award
Individual or group that increased community safety by promoting compliance with traffic laws. Examples include, but are not limited to: seat belt and child seat use, texting and driving, impaired driving, graduated drivers licensing, speeding, etc.

The Salvation Army Chaplaincy Program Outstanding Individual
Person such as a neighbor, committed staff of nonprofits, business or city government, who has made notable contributions to crime prevention. A nomination should detail the circumstances, actions or reasons surrounding the individual's efforts.

Outstanding Block Club/Community Collaboration
Block club/community group with an active involvement in making the community safe.

Outstanding Corporation or Business
Corporation or business that has contributed to crime prevention whether by service, support, programming or advocacy.

Outstanding Faith Based Organization
Faith Based organization that has contributed to reducing crime, fear and disorder in their community. An organization based on faith and/or beliefs, a mission based on social values of the particular faith, and which most often draws its activists from a particular faith group will be considered.
Law Enforcement Awards- Given each year to a law enforcement member or group who meets the qualifications below.

Outstanding Community Liaison Officer (CLO)
CLO that contributed to crime prevention in their community. A nomination should detail the circumstances, actions or reasons surrounding the CLO efforts of community/law enforcement cooperation.

Outstanding Professional Contribution
An officer with distinguished service in the area of crime prevention and community. A nomination should detail the circumstances, actions or reasons surrounding the nomination. When multiple officers are involved in an act worthy of recognition, please list the actions of each officer.

Outstanding Civilian Contribution
A civilian with distinguished service to law enforcement and crime prevention. A nomination should detail the circumstances, actions or reasons surrounding the nomination.
Either Community or Law Enforcement Awards- Law enforcement and community members can be nominated for these awards.
Outstanding Greater Milwaukee Partnership
Law enforcement or community partnerships that have contributed to reducing crime, fear and disorder in their respective community.

Crime Prevention Advocate
Any member of the community or law enforcement that contributed to long term policy, advocacy or education efforts related to public safety.
