Pre Register for Celebrate Smith 2025-2026 Form
----->This form MUST completed for EVERY person who will attend Celebrate Smith.   All forms must be submitted by 3:00 PM on the Thursday BEFORE the Friday scheduled assembly.  All people who are not on the list will need to sign in using the Hall Pass System in the Front Office.  Each name submitted using this form must have previously had their driver's license or passport entered into the Hall Pass System

----->ALL VISITORS MUST SIT ON THE BENCHES IN THE DESIGNATED AREAS--NO STANDING NEAR POD DOORS.  This is to ensure that there is adequate walking space and that these areas remain clear in the event of an emergency.  

----->Upon conclusion of the assembly, ALL VISITORS MUST EXIT THE CAFE THROUGH THE BACK DOORS TOWARDS THE BACK LOOP.  Staff members will be collecting your visitor pass and will check you out of the system.  Visitors are not allowed to follow students into PODs or classrooms.
