All information requested on this form must be completed and accompanying documentation or sources listed as well as letters of recommendation received before April 1st of each year for consideration.  All information submitted shall be retained by the CSHS selection committee.  

The completed application packet must include: (1) Letter of recommendation from the person who nominated you or from another respectable source (please email to jremington@mvusd.net).  (2) Supporting materials which include accomplishments at the school and beyond.
