Quilt Registration Form
Quilt Show Dates: September 30 & October 1, 2017
Deadline for Submission: Friday, September 15, 2015

All quilts need the owner's name permanently somewhere on the quilt. All quilts must have a sleeve installed on the top of the back of the quilt. Miniature quilts may have a 2" sleeve. All other quilts should have a 4" sleeve. Please stitch the sleeve on. Safety pins do not work! Quilts will not be accepted for display without a sleeve. If you have an 'out-of-the-box' quilt to display, please talk to Norma Jean or Peg about the best way to display it.

In the event that there are too many quilts to display, please indicate your priority for hanging with the following notation: If you enter a total of 5 quilts, your first priority would be marked below as 1/5. Do not include charitable quilts that you have already given to the chairs of those committees.

When you submit this form, the information will automatically be sent to Peg Smeltz, msmeltz@antioch.edu. If you have questions or other issues, please call (or text) Peg at 603-762-0012.
