Teacher Referral Form for Augustine Tutoring
Augustine tutors are trained to use the research-based Orton-Gillingham approach to phonics instruction. This method is highly structured, multisensory, sequential, and cumulative. All students, regardless of age or grade in school, start at the beginning.
Please note: This referral form should be completed only by the child's classroom teacher or a teacher who knows the student well. If a parent or other person refers a child, the teacher will be contacted and asked to submit the form.
The Mission of the Augustine Literacy Project is to improve the literacy skills of economically disadvantaged children who struggle with reading, writing, and spelling. Would this child be an appropriate Augustine student?
The focus of the Augustine lesson is on phonics. Does the child have trouble understanding the letter-sound association and struggle with decoding (sounding out words) and encoding (spelling)?
The children we serve are of at least average intelligence, do not have significant behavioral issues, and attend school regularly. To the best of your knowledge, does this student meet these requirements?
If you have answered "yes" to the above questions, please proceed. If you have answered "no" to any of these questions, unfortunately the child is not an appropriate candidate for Augustine tutoring, and we thank you for your interest.
