Use this form to request a Concord Band summer uniform polo shirt or accessories. Due to the early summer schedule this year, we are fulfilling primarily from inventory. Custom orders must be submitted no later than May 5, 2025 so that we can receive your item in time for the first concert. Payment is due when you receive your items.

This is not a fundraiser. The Concord Band offers these items at cost to Band members. We place an order only once a year. You are welcome to submit as many orders as you would like for yourself, family and friends.
