By completing and submitting this form, the parent/guardian gives permission for their child to be dismissed independently at the conclusion of Sunday and/or Wednesday Children's Ministry classes and activities. I understand that once dismissed, my child will no longer be under the supervision or care of Children’s Ministry staff or volunteers. I acknowledge that I am assuming full responsibility for my child’s safety and whereabouts following dismissal.
