Health & Accessibility Survey - SBTS 2026
The intent of this survey is to understand your access needs and how we can meet them to create a safe environment for you to do your work, paid or volunteer.

It will be kept strictly confidential, and will only be reviewed by 
  • the HR Chair, 
  • the Production Manager, 
  • the Festival Manager, 
  • the Artistic Director
(The Chair also has access but does not participate in the review requirement.)

If this team feels that there is information to be shared company-wide (such as airborne allergies), this will be listed without identifaction to maintain your privacy. 

Of course you are welcome to additionally disclose to whom you may wish at any time. 

If this team feels that something below should be shared with your stage manager and/or director, or another member of the company, someone will connect with you to discuss before disclosure.

Physical/medical - If you wish, please describe any physical or medical conditions that you have that may impact your work. 

Examples could include 
asthma or lung conditions; food and/or airbourne allergies; a knee that makes it difficult for you to get up off the floor, needing breaks, space; a refrigerator to express breastmilk, and diabetes requiring you to get snacks, check your blood sugar, or administer insulin at particular times; cannabis use for medical management.
Mental health -  If you wish,  please describe any mental health challenges, or specific topics that relate to trauma for you, that may impact your work. Examples include anxiety that may make specific phases of the process particularly challenging, a past experience of racial trauma that relates to the material in the play, and body image trauma that may make costume fittings challenging.
Sensory -  If you wish, please describe any sensory challenges that may impact your work. Examples include sensitivity to heat, which may need accommodation from the space or costume design, sensitivity to background noise, or parts of your body you never want another person to touch.
Disability -  If you wish, please let us know any disability accommodation you need to do your best work. Examples include, large-print script, parking close to the building, or a support animal.
General life -  If you wish,  please describe any other access that are not covered above. Examples include, you leave your children with a sitter and need the stage manager to keep an eye on your phone in case they have an emergency, you need to leave by a certain time because you use transportation with a limited schedule, or you need to have your rehearsal schedule a certain amount of time in advance to coordinate other aspects of your life.
Safety - If one of the above identified needs creates an emergency, what should your coworkers/supervisors do to best support? Examples include: if you have asthma, where is your inhaler? Is it okay for someone to get it for you? If you miss your bus because we run late, would you rather someone offer you a ride or the production cover an Uber?
