2018 DACTM Educator of the Year Nomination Form
To nominate someone for the Detroit Area Council of Teachers of Mathematics (DACTM) 2018 Educator of the Year, simply complete this form by April 30, 2018.

The nominee must be a current, active DACTM member who is an endorsed and practicing mathematics classroom teacher, professor, or support personnel (e.g. consultant, interventionist, coach, paraprofessional, curriculum specialist, etc).
To nominate you must also be a DACTM member. Self-nominations are welcome.

The Educator of the Year shall be honored at our annual meeting, on May 16th.

In addition to being honored at the annual meeting, the DACTM 2018 Educator of the Year will receive:

- lifetime DACTM membership
- opportunities to contribute to the field via writing, blogging, and speaking at conferences
- wall plaque of recognition
