As a District 11 basketball official, I will not officiate any high school or middle school game without being assigned to it by the SCHSL booking office or the District 11 Director or scheduling designee.

If, after accepting a game, I fail to show up without contacting the District 11 Director or scheduling designee, I understand that I am obligated to pay District 11 Basketball Officials Association the game fee. Multiple occurrences of this type will result in suspension or expulsion from District 11 Basketball Officials Association.

I agree to uphold and observe the Constitution, By-Laws and policies of the SCBOA and District 11.

I have never been charged with or convicted of an offense that would carry more than one year in prison or been charged with a felony. If so, please attach a narrative explanation.
