This form is for students who are registered in a Chang School Certificate Program and wish to officially withdraw from the Certificate Program.
Withdrawing from the Certificate Program does not withdraw you from courses. If you would also like to withdraw from course(s), please consult the Chang School website for procedures and deadlines regarding course withdrawals and refunds.
The withdrawal from the certificate will be processed on your student record as of the date the form is received. This date will display on your official transcript. Forms received after the published deadline to drop in good academic standing each term will not be processed until after that term is completed and will be effective the start of the next term.

If you are in a certificate program and you would like to transfer to a different certificate, please use the Certificate Registration/Transfer link in MyServiceHub (Certificate Program Withdrawal Form is not required).
All certificate transactions (registration, transfer, and withdrawal) are reflected on the student’s academic record and display on the official transcript.
