Owner, as identified below, acknowledges and agrees that Elite Instinct Canine Training LLC, Vom
Kugelblitz Kennels and Training Academy, and any authorized representative of the company is NOT
responsible for any injury, sickness, or illness to the Owner or, his or her dog, may encounter while
engaging the services of the company at any time.
By acknowledging below, the owner agrees to allow use of his or her dogs name and any images or video
taken of his or her dog, in any form or format, for use, at any time, in any form of media, marketing, print
or advertising.
Owner agrees to pay in full for any and all services rendered by the company and that remaining balances
for training services must be paid within 30 Days of purchase.
Owner understands that the Elite Instinct Canine Training LLC, Vom Kugelblitz Kennels and Training
Academy, does not give refunds for services rendered or purchased and not used.
