IMPORTANT:
** Be sure to register for camp/program BEFORE completing this form.
** This form must be completed by a parent or legal guardian of a minor child (“Participant”), or can be completed by themselves if “Participant” is 18 years of age or older.
** Form must be completed within one sitting.
** Have on hand any health, allergy, and emergency contact information, etc. that is pertinent to the participant.
** One form per participant.
** Participants may not attend any youth program(s) without a completed form(s).
