By signing this Waiver and Release of Liability (Agreement), I waive and release Kodiak Baptist Mission (KBM) and American Baptist Home Mission society (ABHMS), it’s agents, servants, employees, insurers, successors and assigns from any and all claims, demands, cause of action, damages or suits at law and equity of any kind, including but not limited to claims for personal injury, property damage, medical expenses, loss of services, on account of or in any way related to or growing out of my presence or involvement at the facility.
This waiver and release is intended to and does release KBM and ABHMS from all liability for damages or injuries on account of or in any way related to or growing out of my negligence, the negligence of third parties and negligence. This is not intended to release KBM or ABHMS from any liability resulting from their intentional conduct.
I further covenant and agree not to institute any claims or legal action against KBM and ABHMS for any claim release by this Agreement. I further agree that should any claim be made against KBH and ABHMS in contravention of this Agreement, including but not limited to derivative claims, I will protect, defend, and completely indemnity (reimburse) KBM and ABHMS for any and claim and expenses including attorney’s fees and costs incurred by in defending themselves or security indemnity hereunder.

I understand that KBM and ABHMS are not responsible for any lost, stolen, or damaged valuables or property.

I acknowledge that I have received and read a copy of the current rules and regulations governing the use of the facility. I agree that I will fully comply with all rules and regulations and with any amendments.

I am aware that while on Kodiak Baptist Mission myself/my child’s photographic image may be taken and used in a photograph/video for the Kodiak Baptist Mission, which may include Facebook, Instagram, and other media outlets. I realize the photograph/video is for information and fund raising. Kodiak Baptist Mission or its representatives may use the photograph video images and edit them to fit the purpose of KBM. No personal information will be released about the photograph/video or disclosed in any other fashion.
Moderate Risk Activity Release
I understand that my child may be participating in activities on and off KBM property that are considered high risk. I feel that my child is developmentally ready, both physically and emotionally, and possesses the skills needed to participate in the activities I have marked on the signature page. My child is in good physical condition and has not had any serious illness or surgery since his/her last health examinations. (There may be additional release forms specific to the activities listed below that need to be signed.)
By signing the moderate and high-risk activity release, I understand the risk inherent in the below activities:
Bicycle riding, boat transportation, campfire cooking, challenge course (low and high elements), Climbing wall, fishing, gymnastics, hiking, horsemanship, kayaking, swimming, tide-pooling, wood shop, and forge, all nature of farm animals. The purpose of the Student Release/High Risk form is to inform caregivers of the risk and to provide the opportunity for their own evaluation of the skills and behavior necessary to safely participate in the activity. You will be asked to sign a separate release form for Horsemanship and The Challenge Course/Climbing Wall.
