MYAA Medical Release
RELEASE AND LEGAL AGREEMENT

I am the participant named on this form (Participant) or the parent or legal guardian of the Participant. I hereby agree to the following.

1. I acknowledge that participation in athletic events necessarily involves risk of physical injury. In return for the Medford Youth Athletic Association’s (MYAA) accepting the registration of the Participant and permitting his or her the voluntary participation in its programs, I hereby release, discharge and hold harmless MYAA, its employees, officers, directors, coaches, officials, volunteers and other representatives from any claims arising out of or relating to any injury that may result to the Participant in connection with MYAA sponsored events, including games and practices, including any injury caused by the negligence of any official, umpire or coach while performing his/her duties during any events.

2. I hereby authorize any representative of MYAA to act for me in a medical emergency. I agree to indemnify and hold harmless MYAA, its employees, officers, directors, coaches, officials, volunteers and other representatives from any claims, including claims relating to the payment of medical bills or fees, arising out of or relating to any medical treatment duly authorized by any MYAA representative.

3. I and the Participant agree that we have read the MYAA CODE OF CONDUCT AND DISCIPLINARY PROCEDURES (CODE), which is available at www.myaa.net. I and the Participant agree to be bound by the CODE. I and the Participant understand that the CODE contains rules governing our personal conduct and disciplinary procedures and that by signing this RELEASE AND LEGAL AGREEMENT we are submitting to those procedures and giving up rights that we otherwise might have to challenge in a court of law decisions by MYAA about our participation and attendance at MYAA events.

WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19 - MYAA
ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT

In consideration of being allowed to participate in Medford Youth Athletic Association events and activities, the participant acknowledges, appreciates, and agrees that:

1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS ,Medford Youth Athletic Association their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event including but not limited to Bob Bende Park, Hartford Crossing Park, Freedom Park, Gill Road Fields, Softball Complex and Bob Meyer Park all in Medford Township (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT
FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP
SUBSTANTIAL RIGHTS BY AGREEING TO IT, AND AGREE FREELY AND VOLUNTARILY
WITHOUT ANY INDUCEMENT.
