BHS Letter of Recommendation Request Form- ORIGINAL
This form is used to record detailed information about yourself so your recommender has information to write your letter of recommendation.
Please be thorough with your responses - your recommender will use the details to write about you. (ex. don't just say you are in Key Club, explain with detail what you do in Key Club (what service/volunteer opportunities you have done. If you are in band, say what activities you have participated in while you were a part of the band, etc.)
  1. It is best to ask your recommender in person or send them an email asking them if they are willing to write a letter of recommendation for you.
  2. Fill out the Google Form.
  3. When you finish, submit your response.
  4. Check your email for a copy of your responses.
  5. Forward your responses via email to your recommender OR print it and give it to them.
  6. Be sure to communicate with your recommender where the letter is to be sent or submitted.
  7. Follow up with them to be sure they submitted it.
  8. Thank them for taking the time to write you a letter - better yet, write them a thank you note!

Athletic Achievements: List sport, number of years involved, Honor or awards earned, special recognition given
Community service/volunteer opportunities: List involvement in any clubs, civic organizations, religious affiliations that might suggest responsibility, organization skills, leadership skills, etc.  (include what you do in being a part of that organization, total hours/duration of involvement, describe the impact this activity had on others (school, community, individuals))
