25-26 OMES SBDM Parent Representative Nomination Form

Every two years, the Old Mill Elementary School PTO is responsible for conducting an election for two parent representatives to the Old Mill Elementary School School-Based Decision Making (SBDM) Council for the following school year.  Any parent with a child attending Old Mill Elementary School during the 2025-2026 school year is eligible to run for SBDM.  SBDM representatives are required to attend a training session (six-hours for a new member/ three-hours for an experienced member) and monthly meetings.

If you would like to nominate a parent candidate for SBDM, please complete this form by Friday March 28, 2025 . The nominee must complete the bottom of the form agreeing to be nominated. Parents may self-nominate.

Should there be more than two nominees, an election will be held in April. The two eligible parent nominees with the most votes will be declared the winners of the election and serve as Parent Representatives on the 25-26 OMES SBDM Council.

I am neither an employee of nor a *relative of an employee of Old Mill Elementary School, nor the Bullitt County Public Schools district administrative office(s). Additionally, I am not a Board of Education member, nor a Board member’s spouse. I do not have a business interest with the district nor Old Mill Elementary School.  If elected, I agree to serve on the Old Mill Elementary School SBDM Council.  Pursuant to KRS 160.380, I understand that I will need to submit to a national and state fingerprinted criminal background check within thirty days of taking office and attending the required training within thirty days of election.

*Relative is defined, pursuant to KRS 160.380 (1)(b), as father, mother, brother, sister, husband, wife, son, daughter, aunt, uncle, son-in-law and daughter-in-law.
