This form must be completed by a parent or carer of the child who is applying. A form will need to be completed for each child applying so siblings cannot share one form. If there are no places available on receipt of your application, your child will be added to the waiting list and you will be notified when a place does become available.
If you have any queries or concerns, please contact Lucy Clark (Youth Theatre Director)
