Patient/ Parent/Caregiver Questionnaire
As we prepare for our Conference and live every day determined to Make It Better for our Osteo Warriors and Families, we ask you to be kind enough to share your experiences and thoughts with us in the hope that we can make this road smoother for those to come. Together we can make a difference.

This survey is in 4 sections- Introduction, Silver Linings, Grey Skies and Recovery. The introduction questions are required (anonymous is a valid response) and the rest are optional so you can share as you choose.

Please share things you learned that were helpful to you, your child and your family during the time of diagnosis, treatment and after treatment. Caregivers are invaluable during this time. If you are a caregiver completing the survey, please consider your answers to questions related to 'your child' even if you are not the parent.
