Prospective Member Checklist
This is an online version of Heartwood's Prospective Member Checklist for use by P2 Homebuyers. It is intended to make it easier for you to complete the PMC and for the Membership team to review it, rather than doing it all on paper.

Introduction
Our membership process is designed to support prospective new members in gaining substantial understanding of Heartwood's Vision, Values, Interpersonal Agreements, our operating structures and processes, and what you can anticipate day-to-day life within the Community to be like.

Upon completing this Prospective Member Checklist (PMC), it is anticipated that YOU will be able to make a much more informed choice as to whether or not Heartwood is the right place to live, for you (and your family, if also present). At that point, the choice is YOURS, that is, you self select: Yes, I/we want to live here and become active members of the Community; or No, now I think that maybe it's not quite right for me.

The "status" of Membership implies both completion of the PMC and actual residency at Heartwood so for P2 Homebuyers, you will become actual Heartwood members once they've completed this PMC and bought your home in P2.

One PMC is required per adult in the household. You must complete the PMC in good faith and inform the Membership Team and/or your buddy once it is completed.

The Membership Team may decide to waive, modify or extend certain requirements due to extenuating circumstances (such as living very far from Heartwood), so long as you have completed the majority of the process, and are able to give full and careful consideration to becoming a member.

Please note that the requirements may be completed in any order you'd like, except that you are strongly encouraged to meet with the Membership Team to discuss membership and your Membership Questionnaire after having completed most of the other requirements.

IMPORTANT: This PMC Google Form has been set up so that you can submit it and then later edit or add responses so that you can fill in your PMC as you complete the various requirements. In order to edit your PMC responses or add more responses, you must save the link to "edit your response". There are two places you can get this link: 1) in the email from Google Forms confirming your submission of the PMC; 2) by bookmarking the link that you can click on just after submitting the PMC. After hitting the "Submit" button on this PMC, the next screen you will see will give you a chance to "See previous responses" or "Edit your response". Click on "Edit your responses" and then bookmark that link so you can return to your PMC later. Unfortunately, Google doesn't give the Membership team any way to retrieve that link. If you don't save it or the submission confirmation email, you'll have to start filling out the PMC all over again.

Please read the following declarations and virtually sign below.
