Thank you for participating in the 3rd annual consumer telehealth benchmark survey for hospitals and health systems. This survey is designed for individuals employed by a hospital or health system who have knowledge or is involved in virtual care delivery. We are defining consumer telehealth as providing the patient choice and engaging them on their terms; through their preferred mode of entry, location and interaction with a provider. In appreciation of completing the survey, you will receive an E-book of the survey results. Additionally, all survey participants that are currently employed by a hospital or health system will receive a complimentary registration to Becker's Hospital Review 10th Annual Meeting taking place April 1-4, 2019. If you will not be attending, you are welcome to designate a hospital or health system employee to attend in your place.

To confirm validity of the survey and to send you your complimentary registration, we are collecting contact information of the participants. However, your responses will remain anonymous.
