Applicant understands and agrees as follows:
1. Permission is effective only upon receipt by Applicant of a copy of this agreement
countersigned on behalf of C&T Publishing, Inc.

2. Permission is granted based upon the above-stated information, which Applicant warrants
is true to the best of Applicant's information and belief. Permission is conditioned upon
and subject to the terms hereof.

3. Permission is non-exclusive and applies only to the specific edition(s) and use of the
Work indicated above. It does not apply to any revised edition, revision, or licensed
edition of the Work. It does not cover any quotations or graphic materials from other
sources that may be incorporated in the C&T publication.

4. No changes, additions, or other revisions of the Material are authorized or permission
without specific prior written approval of C&T Publishing.

5. The required copyright notice and other credits specified below must be printed in a
conspicuous place relative to the Material.

6. Compensation shall be in the form described below. The specific permission fee, if any,
must be received by C&T Publishing, Inc. within thirty days after the date of this
agreement accompanied by a copy of the counter-signed agreement. Payment should be
made payable to C&T Publishing, Inc., and sent to 1651 Challenge Drive, Concord, CA
94520-5206, USA. Any permission granted herein is void unless C&T receives
compensation in accordance with this section.

7. If the Material, plus any material C&T Publishing or its other subsidiaries also included in
the Work, amounts to a total of 10 percent or more of the Work, then the specific fee
automatically becomes an advance against a royalty to be determined by C&T Publishing
based upon information furnished by Applicant.
8. Permission will cover use of the material in a Braille and/or large-type edition of the Work
for the benefit of the visually handicapped but only if such editions are sold or rented for
nonprofit purposes.

9. Upon publication, C&T Publishing shall be furnished one copy of the Work at the above
address sent to the attention of Valerie Palacios, Publicity Manager.
10.Permission shall automatically terminate if the specified edition of the Work is not
published within one year from the date of this agreement or if such edition remains out of
print for six months or more. In the event of such termination, C&T Publishing shall be
entitled to retain all amounts previously paid and to be paid any sums owed under the
terms of this agreement on the date of termination.

11.The term of this permission is on year from the scheduled publication date of the Work as
referenced previously in this agreement. The permission shall automatically terminate
without notice if this agreement is violated, including failure to issue specified
compensation or to include the required copyright notice.
