Self-assessment declaration *
I confirm that I have undertaken self-assessment  checklist and confirm that I do not have any of the following symptoms: high temperature (above 37.8C), new continuous cough, shortness of breath, sore throat, loss of or change in normal sense of taste or smell, feeling generally unwell or have been in close contact with / living with a suspected or confirmed case of COVID-19 in the previous 2 weeks.                                                                                                                                                                                  For players below the age of 18, the information must be completed by a parent/carer.

I have completed the EHA registration document (Note: this only needs to be done once, but must be completed 24 hours prior to initial session or game).
Actions required prior to entering the pitch area and during activity
1. All participants should complete a health self-assessment (see above).
2. Sanitise hands.
3. Maintain social distancing, including when depositing stick bags and equipment.
4. Do not share equipment, including face masks.
5. Enter and leave the pitch via the designated entry and exit.
6. Do not shake hands.
7. Avoid shouting where possible and remain respectful of all participants.
8. Avoid using dug out areas, other than for first aid or disciplinary matters
